A husband, as a stress factor!!

There she lies all alone,so frail."As white as a sheet" really describes her.She was admitted with diabetic keto acidosis, which is a state in which the patient's blood glucose goes sky high with acetones in urine. She also had DVT of both lower limbs which is resolving now.
The fact that there is no one at all to visit her in the ward and to take care of her always bothered me.Now i came to know that she has a husband, who comes once in a while and shouts and upsets her and leaves. If there are people like her husband in your lives, i tell you, you are better off all alone.Seriously!
We have shifted her to the Medicine ICU, cos her DKA has returned and the blood suger level just wouldn't come down below the 300's for the past two days. Yes, we are looking for any source of stress and infections, and they are as for now ,none. She is so young, just 27 years old.The only stress we can find is her no good god-damn him husband!!!! *scowls!!

Need MDs at studios

Two days back i walked in to my room to see that my room mate has paused one of the episodes of the One Tree Hill series to a scene where it was showing a hospital examining room. What caught my eye was the x-ray that was mounted on the viewing box.It was mounted the wrong way.Left was to the right.I believe that x-rays are marked so that it will be known which side is right. But i also believe that these english series really need an MD to give some medical advise when they are propping up for these kind of scenes.

The series that tops the One Tree Hill episode is a Desperate Housewives second season episode.(Oh,yes i watch them and i am a loyal fan.)They had the x-ray upside down. There Gabby is, listening to the doc's advise while the x-ray is upside down at the back of the scene...*teeeheee!!! :):D

Lice Nightmare

Patient: Doctor, i have got these enlarged lymph nodes in my neck. They are painful.
Me: Let me see,how long have u been having them?
***I went in to a detailed history.
Patient:oh,just a month or so...(and so she goes answering my questions)
Me:Let me examine your neck and axilla please. Please take off your head covering.
(I went on examining for lymph nodes and found enlarged tender right deep cervical lymph nodes.Axilla and supraclavicular area were free.
Just when i finished examining..
Patient:And doctor, i have these "things" in my scalp.I cant seem to get rid of these "places".
I went and examined a spot she indicated with her index finger.Tried to seperate hair strands from the tightly tied back hair.I saw flaky seborrhic dermatitis from the look of it.I was about to do some more looking at her scalp when i noticed them....Crawling liesurely along her hair.As if they belonged there.
I took a step back and placed my right hand at the rim of the wash sink near the examining table and the other on my hip.I looked nonchalant as i told her that she has got lice infestation.
I discharged her after prescribing the antibiotics for the lymph node inflammation with advises to follow up and with a referal to the dermatology clinic in her hand.
Point to ponder: I haven't seen lice infestation ever since i left primary school and it was a bit of a shock to see them, those bugs so close.Brought back the "lice nightmares" i had to endure during my childhood. The more my mom cleaned my head from them, the more i got them from school buddies.

Very seriously..

As a surgical intern, i have come across many cases that has been complicated by Diabetes.
Among them is the Diabetic foot.Diabetic foot, in common terms occurs due to pus filled collections occuring in the leg or foot of a person who is diabetic.He/she becomes resistent to treatment (or neglects the condtion) and the infection starts spreading and eating the tissues of the leg.In order to stop the spreading of the infection, surgeons need to amputate the limb with strict controlling and monitoring of the patient's blood suger level.
The pain and grief the patients and their family goes through an amputation, i have witnessed and stood by their side through it as their doctor. I have seen how the morbidity affects them in so many ways. This is just one of the complications of the disease.
Then again, there are those complications that can occur and which is not readily infront of our eyes to be seen like the above mentioned diabetic foot.I would like to mention some of the complications such as kideny affection, deterioration of eye sight and peripheral vascular disease.
My intention is that anyone who reads this post to think about their relatives or friends who are diabetic and to always encourage them to check their blood suger levels to keep it always controlled,to always do regular check ups with their physicians and ask them about doing regular kidney function tests and fundal examinations.Another important thing is not to ignore any sort of pain in both legs.Get medical help and rule out any underlying treatable medical condition. And last but not the least, tell them to always take any infection very very seriously.
...very seriously.

..broken future

I dont know why i think of this girl whom i met while i was in secondry school.Actually i met her at the National Library.Back then, the place that housed the National Library wasn't much spacious.I dont believe it is more spacious even now also, as it shares the building space with the British Council and some Art Gallery.Anyway, I happen to always run it to this specific memory of this young girl of age 12 or 13 at a very odd time. While i am making roshi!! Can u imagine that??

Anyway,i first met her there in the library, which was the exact time i became a book worm of some sort.I spent all my waking hours with books. She was a nice looking girl, but from her clothes i came to realize that there wasn't anyone seriously taking care of her.She was really interested in reading, although she didnt know much of english at that time and asked a lot about books i was selecting and showed me some of the books she liked.I then asked her which books were she going to borrow, and she said that she was not a member.Oh,and with that she ran off saying that her grandma will be looking for her now.I didnt think much of this at that time, but after a few weeks, i ran into her at a friend's house. This house is situated just a few blocks away from the library. This was the first i visited my friend and her family was renting the space at the back of the house. The little girl was happy to see me and showed me her living space which was very shabby.I found out that day from her that she is living with her grandmother, and doesn't have her mother or father taking care of her.Her parents are divorced and both of them are married to other people and living a life of their own.
This was the last time i met her.Well, actually face to face that is. After some few years, i started seeing her on TVM as an actress in some dhivehi songs. She became pretty famous in a very short period of time, and then disappeared from that scene too.
I keep wondering about her and what became of her future. The girl went from being really interested in books to become a cheap dhivehi songs star.Given the opportunity, i am sure she could have become more. She could have become an educated woman with a bright present and a much brighter future.I wonder how she is now.Whether she is taking care of some few toddlers of her own and coping in a world that came her way inspite of the dreams she had for a brighter future when she was young.I wonder if she's got a caring husband.Or if she is married at all for that matter.Or whether is she leading some dark life of her own.
I just keep wondering.....
