In the April 12th issue of Stroke ASAP, the authors have stressed upon using of recombinant tissue plasminogen activator with in 3 hours of onsent of stroke symptoms. I wonder whether this is available in Maldives.&lt;br /&gt;The other changes that came in to practice guidelines was the changes that came to the Cadio-Pulmonray resuscition sequences. I wonder whether IGMH has a resus team who responds to all arrest cases like they have here or elsewhere in the world?! I believe its very important to to have themedical team trained and refreshed constantly about CPR so that all can work more efficiently in an emergency life threatening situation like cardiac arrest.&lt;br /&gt;&lt;br /&gt;By the way, i have seen "Pirates 3". Wow!! It was one big adventurous ride.The highlight of the outting was me having a perfect classic cinabbon and a tropical blast at &lt;a href="http://www.cinnabon.com/home.html"&gt;Cinnabon&lt;/a&gt; while watching shoppers walk by, parents being dragged by their kids and vice versa, kids falling while running after runaway balloons. Then again i hit the ground with rapid acceleration when the idea that i will be responsible for lives hits me.That thought brings me down to earth alright.&lt;br /&gt;I have been busy in Pediatrics and particularly in NICU the past two months and couldn't blog.All i do when i get home is get some food and sleep and sleep.Until its time to go back to hospital.Life all of a sudden seems to be very predictable for me. Isn't that boring??It's supposed to be,but  i didn't get time to think about it until now.&lt;br /&gt;Now that i have some free time, i did some normal girl stuff.Went to the mall,shopped a bit, watched "Disturbia". I say it's one cool movie. I like the kid in it.What's his name again?Hmm.. can't remember. Now i want to go and see "Pirates" third part. I love Johnny Depp and the piraty and unhygienic look everybody has in it.In how many years did the pirates took a shower those days?  :-)&lt;br /&gt;What am i blabbering about here today??!!! To ask them about their last menstrual period (LMP). But this concept clearly escapes doctors a lot and mistakes are being made everyday which can affect the outcome of pregnancies.I remember this particular incident which happened during my CAT days in IGMH. I was assisting an Indian doctor that day and so there came Mrs.X with her husband to us about a vague abdominal pain she was having since one week. She looked generally well and she doesnt seem to be in severe pain and she described it to be vague and occasional.The doctor asked a few questions,and asked me to give the patient a request form for an abdominal x-ray.After a few mins,Mr &amp; Mrs.X came back with the x-ray and lo &amp;amp; behold what a sight greeted us!!&lt;br /&gt;Through the x-ray staring at us ( more like screaming at us) was a single fetus of approximately of 18 weeks old with it's cute umbilical cord,eyes and fingers nearly visible. I was horrified. I doctor's face went blank. She made some stuttering apologies and blamed the patient for not telling her that she was pregnant. Mrs.X said that she had no idea that she could be pregnant. The doctor cunningly kept the x-ray with her and sent off the patient telling her that she has to follow up with an obstetrician. Since that day, this particular incident has occupied my thoughts during odd times. What was this doctor thinking? What was her preliminary diagnosis? Intestinal obstruction? Anyone who deserves to be called a doctor should know the signs and symptoms of intestinal obstruction and as i remember well,the patient was not presenting with any of it. She didn't even ask for the correct view of abdominal x-ray needed for such a case.&lt;/div&gt;&lt;br /&gt;&lt;div&gt;&lt;/div&gt;&lt;br /&gt;&lt;div&gt;Meticulous history taking and a simple lab test will rule out any doubts of a pregnancy and will save doctors facing situations like this. This is the way we are taught in medical school and some how, most of us forget what's important during the course of time. Those were happy memories for me because with those books i was with Finn and Sawyer all the way through their adventures. I felt like i was right there with them and witnessing everything they did first hand with them.Those days i won't put a book down unless i have read it to the last page of it.That much a book worm i was then.My dad sometimes will order me to sleep around 11pm or so, but still i will switch on the reading lamp and will keep on reading. The lack of articles on different dhivehi websites about current health issues in the Maldives. Articles or web links which provides the readers information about a certain disease that's spreading around and how to prevent and treat etc etc aren't practically non existent.I was surfing the other day and checking out various sites, which i thought might have links providing such information.I was specifically looking for anything related to Chikungunya Fever.But i didnt find any such links or articles.I checked out the haveeru website through which i couldn't find anything about it.Finally i tried to access the Ministry of Health website.The freaking web page was refreshing by itself every 2 seconds and i couldnt get to see anything on it. Their web master must be in a coma or something!What a shame on the health system!And to the goverment web network in general!!!!Shame on them!!&lt;br /&gt;I believe that these websites should offer information relevant for the normal population about such epidemics.Concuring an epidemic through educating the mass population is vital.&lt;br /&gt;&lt;br /&gt;Oh,i gotta run now.. type='text'>Clothes up,pull off,remove....or whatever!</title><content type='html'>&lt;span style="font-family:trebuchet ms;"&gt;The importance of having patients remove their street clothes and wear some sort of loose gown for physical examination is being ignored by most medical institutions in the third world countries.I can say about this country where i am in right now, that it is not something anyone ever heard of. Take your clothes off for your doctor? NO Sir! Or Ma'am!&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Trebuchet MS;"&gt;&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Trebuchet MS;"&gt;But if you are a doctor, you will know how difficult it is for example, to auscultate (listen) to one's lungs &amp; heart with tight fitting clothing on.Winter makes it worse because every patient comes with layers and layers of clothing on,and i will be asking several times to take up, pull off or something like that for each layer i encounter with.This becomes more or so a "struggle" on my part while i try to auscultate huge female patient's with pandulous breasts on a very narrow examining table.I guess examining tables were made with europeans size and built kept in mind.I usually end up fumbling and struggling to auscultate the apex of the heart with the constricting clothes half way up which won't go up just a few more inches so the patient's breast won't be blocking that exact spot i want to put my steth on.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Trebuchet MS;"&gt;I believe that IGMH and goverment hospitals in Maldives should start keeping gowns in the examining rooms, so doctors can provide patients with gowns when the need to be completely free of street clothes arise when the doctor and patient comes to an agreement to do so.&lt;/span&gt;&lt;div type='text'>An unnecessary death</title><content type='html'>I dont know how to write about this.Its too mind boggling right now.The death of a patient on my on call last night.She was suffering from chronic renal failure and finally she had her second dialysis yesterday with a blood transfusion after which she started to complain of severe breathing difficulty.Anyone who knows about chronic renal failure will know that so many factors can be involved in that kind of breathing difficulty.But after i examined her i came to the conclusion that she was having impending pulmonary edema and the residents needed to take action right away.&lt;br /&gt;But no one budged.It was an untimely death.She died after a few mins on her way to MICU which wasnt even 100 foot away from her bed.Her son and daughter beated down the ICU door and ran after the doctors to beat them up and the hospital security had to come up and control the fight and screaming.It was a very heart breaking death for me because as i believe it, it was an unnecessary death.A death that could have been prevented if the residents didn't ignore the complaint and my report on her.&lt;br /&gt;It is unacceptable for patients to die because of doctors ignoring them.Death due to mistake is better than that.Last night, the residents responsible for her practically hid from the patient's relatives who were looking for them for help.There are patients who come at the middle of the night with nonsensical complaints but it is our job as doctors to varify them and explain and make them understand the situation and breathing difficulties.&lt;em&gt;*huff puff!!there i went prescribing nebulizers &amp;amp; ordering ABGs and running from my counter to his bedside every 5 mins.&lt;/em&gt;&lt;br /&gt;&lt;em&gt;-&lt;/em&gt;Afternoon visiting hours gave us a fist fight between two relatives who were visiting.Had to bring up hospital security for that and had to send down the injured to the ER.*&lt;em&gt;arrghhhh!!!&lt;/em&gt; and i was busy seeing patients on the upper floor.&lt;br /&gt;-The resident ordered a set of ABGs for so many patients and forced diuresis for that many and in the end no one collected the urine after the forced diuresis drips were over even after careful repeated instructions by me and the resident.*&lt;em&gt;bangs my head in frustration! &lt;/em&gt;&lt;br /&gt;-The resident realized that we have a patient who needs a change of dressing of her leg ulcer at 2am in the morning and wants to call the surgical department.*&lt;em&gt;how to my room to see that my room mate has paused one of the episodes of the One Tree Hill series to a scene where it was showing a hospital examining room.What caught my eye was the x-ray that was mounted on the viewing box.It was mounted the wrong way.Left was to the right.I believe that x-rays are marked so that it will be known which side is right.But i also believe that these english series really need an MD to give some medical advise when they are propping up for these kind of scenes.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;"&gt;&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;"&gt;The series that tops the One Tree Hill episode is a Desperate Housewives second season episode.(Oh,yes i watch them and i am a loyal fan.)They had the x-ray upside painful.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;"&gt;Me: Let me see,how long have u been having them?&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;"&gt;***I went in to a detailed history.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;"&gt;Patient:oh,just a month or so...(and so she goes answering my questions)&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;"&gt;Me:Let me examine your neck and axilla please.Please take off your head covering.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;"&gt;(I went on examining for lymph nodes and found enlarged tender right deep cervical lymph nodes.Axilla and supraclavicular area were free.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;"&gt;Just when i finished examining..&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;"&gt;Patient:And doctor, i have these "things" in my scalp.I cant seem to get rid of these "places".&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;"&gt;I went and examined a spot she indicated with her index finger.Tried to seperate hair strands from the tightly tied back hair.I saw flaky seborrhic dermatitis from the look of it.I was about to do some more looking at her scalp when i noticed them....Crawling liesurely along her hair.As if they belonged there.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;"&gt;I took a step back and placed my right hand at the rim of the wash sink near the examining table and the other on my hip.I looked nonchalant as i told her that she has got lice infestation.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;"&gt;I discharged her after prescribing the antibiotics for the lymph node inflammation with advises to follow up and with a referal to the dermatology clinic in her hand.&lt;/span&gt;&lt;br /&gt;Point to ponder: I haven't seen lice infestation ever since i left primary school and it was a bit of a shock to see them,those bugs so close.Brought back the "lice nightmares" i had to endure during my childhood.The more my mom cleaned my head from them, the type='text'>Very seriously..</title><content type='html'>&lt;span style="font-family:courier new;font-size:85%;"&gt;As a surgical intern, i have come across many cases that has been complicated by Diabetes.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;font-size:85%;"&gt;Among them is the Diabetic foot.Diabetic foot, in common terms occurs due to pus filled collections occuring in the leg or foot of a person who is diabetic.He/she becomes resistent to treatment (or neglects the condtion) and the infection starts spreading and eating the tissues of the leg.In order to stop the spreading of the infection,surgeons need to amputate the limb with strict controlling and monitoring of the patient's blood suger level.&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;font-size:85%;"&gt;The pain and grief the patients and their family goes through an amputation, i have witnessed and stood by their side through it as their doctor. Can u imagine that??&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;font-size:85%;"&gt;&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;font-size:85%;"&gt;Anyway,i first met her there in the library,which was the exact time i became a book worm of some sort.I spent all my waking hours with books.She was a nice looking girl, but from her clothes i came to realize that there wasn't anyone seriously taking care of her.She was really interested in reading, although she didnt know much of english at that time and asked a lot about books i was selecting and showed me some of the books she liked.I then asked her which books were she going to borrow,and she said that she was not a member.Oh,and with that she ran off saying that her grandma will be looking for her now.I didnt think much of this at that time, but after a few weeks, i ran into her at a friend's house.This house is situated just a few blocks away from the library.This was the first i visited my friend and her family was renting the space at the back of the house.The little girl was happy to see me and showed me her living space which was very shabby.I found out that day from her that she is living with her grandmother, and doesn't have her mother or father taking care of her.Her parents are divorced and both of them are married to other people and living a life of their own. &lt;/span&gt;&lt;br /&gt;&lt;span style="font-family:Courier New;font-size:85%;"&gt;This was the last time i met her.Well, actually face to face that is. Nooo!! Broke anything? Of course not!!Did she umm share jokes with other interns about the hat the director wears? Accidents and emergencies department.Well, i have being waiting for this rotation all this time.&lt;br /&gt;Oh,by the way, that fat lady with the hypertrophied vocal cords doesn't yell at the patients as much as she used to.Maybe she sensed that this foreign doctor doesn't like her blaring voice.
